After contacting Bob Kvederas Jr. on what I labeled a "unlisted combo number", his reply was that his father, Bob Sr. believed it to be a broken #4 and not a #1 reverse.
Upon very close examination, and comparing the notes with the #1 and my note, I noticed the genuine #1 shows a small horizonal line on the top of the #1, whereas my note displays a very small angle on top (kinda like the #1 posted here).
With that said, I just updated my 2004 Handbook on page #53 (F-N Block), and added a comment to the plate combo 5/4 with the notation of the "broken 4" variety..
I guess I answered my own opinion question, and at the same time if I asked anyone else that knew nothing about a broken 4, what number do they see, I would bet their answer would be a #1...
