I think it just means the D was punched 2 additional times. Once South and once Southwest.

There are 1000's of different RPMs (Made from different dies) and thus have a number according to the die number listed. There are several different systems used. CONECA/coppercoins/-Wexler/Crawford numbers. But the 100 are the ones that CONECA picked as the strongest 100 dies. So to reach a top 100 dies of the RPMs would mean that it is considered the strongest examples of the different RPMs. Here is a list of the Lincoln RPMs that have a few images to help you appreciate the scope they cover.
