Smoothed is a term often used in EAC coins. Usually it involves moving the metal around to smooth out a deep gouge or hit, that has disrupted the field. Sort of like a really hot iron pushing metal back into the gouge to fill it up and make the coin smooth in the problem area.

PR62RB? Looks like a details coin to me, always buy the coin not the holder. I wouldn't want that coin in my collection either, and if I was buying it as a coin to flip, I'd be in around $65.00-75.0 at most, then selling at $95.00-115.00.
