Malaysia has its own mint, owned and operated by the Central Bank of Malaysia. They've been making their own coins sine at least the 1980s.

As for the authenticity of this piece, I'm thinking surely it falls under the category of "mint sport" rather than "mint error". Something like this being created by accident without somebody standing there making it happen seems awfully improbable.

I like it - really interesting!

About 5 years ago, a huge batch of Malaysian error coins appeared in the market. Since then they have pretty much disappeared. As of what happened back then, there was huge suspicion that such coins were created under 'mint sports'. In particular Malaysian coins dated around 2005 - 2010 are suspected to fall around this time.

That said, it is a lot harder to find error coins beside this time frame.

I am surprised that they struck them so close to each other and not reverse one of the strikes..
Awhile back I posted a 2007 quadstruck 50 Sen that had one strike flipped over, with all strikes struck off center... I did find it interesting to have a Lettered Edge (that tells me the edge lettering is seperate and not part of the collar).
I stopped purchasing these on ebay when they started to flood the market...

As for all the error coins I have from India, I believe their quality control is poor at best... My belief is they would allow minor errors to go through and enter circulation, instead of shutting production equipment down for repair and slow down production output...
