Ran across this one on ebay. Any thoughts on what might have caused it. Listed as "Very Rare" of course.

It's a STABWOS. Struck Through A Big Wad Of Something.

Sorry, couldn't resist. In essence, though, that is probably close to the reason for the blocked strike. I'm guessing something was on the anvil die or it was a bad planchet. But I'd go for the former.

Our experts may be more knowledgeable about such an extreme case.

Note how the devices widen in that area? That is an indication of a Struck Through Grease. When the devices get smaller, then it is a case of a reduced die. Not a die variety. The die when it was new was complete.
