Check the weight, most likely struck on a tapered and damaged planchet.

biokemist6, I think you are on to something... the weight is 2.73g and the thickness in that area is 1mm

I was thinking planchet error as well, given the weak strike in areas and whatever those holes are. The weakness on the obverse is over the hole'd areas on the reverse.

The reverse looks like a mild lamination also.
