Can someone tell me if this is a mint error or if someone has tampered with this coin. Please also note the extra wide rim.

The nearly-missing digit is likely the result of grease clogging that area of the (incuse) did. Often called a " Greaser".

Can you tell me if the wide rim has any significance or is it something that happens with wear?

To me, it appears the nearby scratch may have affected the coin. I don't really know for sure.

to CCF. The weak digit looks like a Grease Filled Die along with circulation wear. The rim looks normal for a well circulated coin.

In my opinion, the scratches have nothing to do with the weak date digit.

The thicker rim is from circulation wear/flattening. The deep scratches on the coin turn this one in a cull.
