How could I name this kind of error?

I would guess severe die damage.

Maybe the feeder finger has broken off, or it's feeding extension movement has been greatly increased in error.

I would also guess the minting press would have automatically stopped very soon after this coin was struck, thus making this individual error type particularly rare.

Wow! That's something.

I don't know what that is -- maybe 'embedded matter'? More information here: http://www.error-ref.com/?s=embedded+matter

But that is not a really good match.

sadly, your coin has been severely damaged..nothing was in between the die and planchet to make this since you can see some remnants of the design. also, note how the rim is affected..this is from metal being forced out during whatever caused this damage. if this happened in the striking chamber, the collar die would prevent the rim from going out of round at all.

Because of the lighting, this is incuse on both sides. If raised, the highlights and shadows would be reversed.

Another thing I saw on closer examination. The difference in depth and the interrupted horizontal line of the anomaly on the arc of the 5, to me, points to a depression and not raised metal.
