The loop in the number 6 seems to have a broken die chip. Also some of the devices appear to be doubled. I hate using the D word! Any advice is appreciated!

There maybe some chipping on the post that would create the center part of the '6'? That is probably the case on this one. If it were Grease Fill the digit would be affected. as it is from what I can see is that the post was affected.

Looks like most of the coins problems are from Die Deterioration.
