@CuAu just trying to learn here--not trying to cast shade on you or these amazing pieces. Please bear with me.

These seem like the sort of thing that mint workers would do on their lunch hour as opposed to legitimate manufacturing errors. Based on the posts on CCF, I feel like the Canadian mint puts these out with some regularity, but less often the US mint. Do you think that these pieces are the result of similar shenanigans?

I'm not too sure about that myself. To me I think it's a combination of coming to work hung over or tired and not caring, Shinanigans, machine malfunction, maybe even they intentionally made them to make money, because most of us think we aren't paid enough? Maybe also laziness..

The odds on a 90% O/C strike showing the date and MM versus a regular strike must be astronomical.
