1996 Lincoln Cent Errors Rim And Face/Neck

There seems to be something incorrect in the way it was made. I'm not sure though. From his nose to his lower neck and then there seems to be an extra chuck on metal on his eyebrow.

Would something like a coin roll machine cause the rim to bubble up like that?

Eyebrow looks like PSD,face looks like MD and rim looks like worn die issue.

The "second rim" (both) are metal flow/die deterioration later die state issues. A weak strike and/or grease filled and polished dies make details indistinct. Welcome to the quality controls of US mint on stinkin zinkin planchets and the new fangled technology of "single-squeezed" coin minting during the "Decade of Progress"...snort...sigh
