1972-D Cent MD Or The "D" Word? Mintmark Question

I believe this is MD but looking at some of the references the pictures for legit DDO sometimes show flat doubling.

My main question is about the mintmark, was it applied by hand using a punch and hammer?
If so, from what years until it was integrated into dies?
This one looks as thought another tilted "D" is inside the top "D".

I am trying to understand why the OMM, RPM, etc and how they all happen.

All the devices are showing the same thing. It is from doe wear flowing towards the rim direction.

Ask and you shall receive, punching in a mintmark on the die. This is done on each working die used, Denver and San Franscisco. To under stand read Vol. 1 below and also check out the site http://www.error-ref.com for a great description of errors and the processes.

You should include the date, mintmark, and the denomination in your titles.
