The collar is the only thing keeping this Cud in place. Eventually it comes off and the die will have a Cud and then be retired.

Probably the worker running the machine hears a loud noise. I don't remember seeing as many reverse Cuds as the obverse Cuds. Here is one that han't come loose yet, but note how the collar is keeping it in place on the coins struck with it:  Here is a coin with a Cud on both sides of the coin:    This is a big die chip that the reverse would not strike up on it:  Because of the missing obverse part of the die, the reverse would not strike up. (No resistance to form reverse area)
