Looks like a normal coin to me. Note the devices are all normal in size. The big DDO for this year/mint is this one. 1995-D DDO-003

Not a Doubled Die. This is Die Deterioration Doubling that you are seeing on the date. It happens when the die is worn out (Very Late Die State). Just a normal coin.
