Those are called plating bubbles, trapped gasses or debris between the plating and zinc core, is corroding zinc underneath. Will form lines and/sometimes noticeable corrosion. The effect seen on the D is also seen on date, this is generally known as " DDD", Die Deterioration Doubling,where later die states metal flow has begun moving the die metal outwards, effects are seen towards the rim of the coin.

There is also an issue with split plating under the mint mark. (SE)

The area SE of the mint mark can't be an extra 'D'. Do you know why? In 1990 forward the mint mark is part of the design. There is no way there can be an extra 'D', because the mint mark is no longer punched after that year. To our brain it looks like something, but it really isn't what our brain tells us what it is. While it looks like a 'D', that area is not raised. It just a trick our brains pull on us.
