This looks like a "knock-off" of some kind of this medal (also dated 1721) which sold for over $22,000 in Jan 2015.

This one is for sale now for $4k. More like yours. Neither have the Alexander obverse.

Hopefully someone who has experience in detecting forgeries will come on and comment.
For the 1804 pattern rouble, some details of your coin do not look correct. Some of the lettering looks different (in the word AVTOCR for example, where the O and R do not look right). The folds in the emperor's neckwear are not the same as the coin in the link, etc.

There is little doubt in my mind that your medal is not authentic. It closely resembles this admitted counterfeit copy sold by a large Chinese website known for selling nothing but counterfeits.
