It has been removed manually. PMD The lack of rim on the reverse indicates that. You cannot have a rim only on one side. Does have a slight MAD though. There was a similar coin on an earlier post but I can't remeber where it was.
It's Post Strike Damage (it happened after it left the U.S. Mint). It's not an error. The reverse has been sanded off with a grinder. It's worth face value unfortunately.
