Is this coin real? please ignore the fact it has slabbed as there been cases where fakes have been slabbed.

@AlRashid, yes you are correct that fake ancients have been slabbed and we have had several conversations here in the past about how in general the authenticity is not being guaranteed by the TPGs. With that said, I would be inclined to trust a slab over our opinions here as the TPG had an opportunity to measure dimensions, weigh, and XRF/scan the coin to ensure that these parameters were appropriate. About the best we can do here is compare these images to those of known fakes and try to find a match.

By no means am I an expert on ancient coins but comparing a photo of a genuine Alexander Posthumous coin, It appears to me that there are many differences that would make me question the authenticity of the coin you posted.

Perhaps one our more knowledgeable members could give an opinion.

The Alexander coinage was struck at nearly 100 mints over more than two centuries. Variation is expected. While a tiny number of NGC certified coins have turned out to be fake, your chance of getting a fake coin in an NGC slab is far less than the chance of getting a raw fake, even among top-tier auction houses. Personally, I dislike slabbed coins but my dislike has nothing to do with guarantees of authenticity.

I think that we had a thread a while ago concerning the fact that some of the grading companies do not guarantee the authenticity of ancients. However, the slabbing does give you high confidence nonetheless.
