Appears to be a classic 1961 D--some wear and environmental crud. Photo of LIBERTY is a little out of focus. At first glance, the B appears to have taken a hit while the coin was in circulation, causing it to look 'smudged'. A clear photo would help. Others will weigh in with their comments.

That's just pushed over and flattened metal from circulation wear and tear, which is Post Strike Damage ( PSD), not an error.
