Nice clipped planchet. It would be my understanding that the opposing dents in the middle of the clip and opposite rim would be from a foreign object chain strike. But I am open to other explanations of course.

The flattening on the other side of the rim is Blakesley effect

Maybe wrong, but thinking the raised dents or material on reverse occurred after strike. Maybe jammed up a vending machine at some time.

I was guessing on the chain strike based on Mike Diamond's comments on my double struck Oregon quarter. You can read his comment in that thread and see if I am way off or on track.
