@antmark3d, for the moment, I'd believe damage over a brockage as you have displaced metal on the rev, but a potential pressure point on the obv. Something squished the stars and date for sure. Is the diameter a bit over normal?

It has the same diameter of a regular nickel but thinner...hope that helps.

My feeling is it is PMD, but that's mostly based on the punch mark in the middle of the obverse.
