Image is unclear so all I can really say is try to get better images.
From what I see from the unclear image, all it is is environmental damage and PMD, Post Mint Damage. Nothing unusual, you just found a really beat up coin.

to CCF. I agree it is a post mint damaged coin. How it happened is any ones guess.

It looks mangled. It may not be deliberate. It may have just been caught in a piece of machinery. But of course it doesn't really matter.

It wasn't done during the striking of the coin so it can't be an error.
Some form of PMD. Only worth a quarter if someone will accept it.
