Looks like glue or foreign substance on it to me (possibly solder).

I had a thought of it being a huge Interior Die Break, but I am very skeptical and I think that it's Post Strike Damage.

There is a star imprinted on the error area next to the ear glob. Quite interesting.

Looks like PMD to me. I'd soak it in 100% acetone to see if it comes off.

I believe the unusual pattern is actually a reverse of the New York State Quarter, near the top of this image -- even some of the rim is apparent, from the nose to the ear:
