1979-D Quarter Dollar 2 Minting Errors. Stamped On Top Of Another Wei Coin?

I found an odd quarter with 2 obvious minting issues, but it also appears to be stamped on top of some other weird type of coin. Has anyone ever seen anything like this before?

There is a 4 that is easily visible to the right of Washington. It is not punched or engraved on top. Depending on the angle it is held at, the details of the 4 show up more clearly in the background. There is also a few other oddball details that show up along with it. On the reverse, there appears to be the the corner of a cross peaking out over the shoulder of the eagle. It's wierd and despite it being quite obviously - well circulated - these odd ball details made me stop and take take a closer look and I'm both baffled and intrigued. Mostly, I'm wondering if anyone has seen something like this before - and if you have any idea what it might have been stamped over? Ok no prob--these pics will do just fine. I see that the mintmark was struck with a broken post on the die. That is a fairly common occurrence, but happened at the mint. The letters I and N are somewhat smushed together, but based on the wear on this coin, that may be just due to damage. With regard to the other four areas circled, the only one that comes through somewhat plainly in these pics are the feather-type objects above IGWT. At first, I was thinking that perhaps you had a die clash here, but I'm not sure that this would line up correctly. The best way to check that is to create a digital overlay of your coin and see if the alignment matches.
