I always was under the impression that coins incapable of attaining a numerical grade, received 'details' verbiage on the front of the slab.

Is this something commonly done only if requested?

As an example of the aforementioned issue what would be the advantage or disadvantage of having a coin labeled similar to the:

1858-O 50C Misplaced Date, WB-108, FS-301, WB-108, Die Pair 7, R. -- Environmental Damage -- PCGS Genuine.

I have also had this question - at what point is a coin so bad that it just gets genuine and no approximate details grade? I have a PCGS classic head double eagle that is genuine not gradable for code 98 tooling but it looks BU to my eye. So it would be easy to give a rough grade with details but for some issues they won't do it.

Through PCGS, this can be done on request. PCGS attests to the authenticity, but does not grade the coin.

Would any top TPG label this coin genuine and with a letter-only grade which this example does not have?

Well I thought I had an answer but it may not hold up. When PCGS first started slabbing problem coins they only put GENUINE on the label, no details, no details grade just GENUINE. The problem is this is not one of the early Genuine slabs, it is a recent slab. I wonder if it could be a reholder of one of the early Genuine slabs and they just copied the label over from the old label?

My guess is that the variety designation took up the space on the label and their labeling system can't handle both simultaneously.
