Well for sure it is heavily worn and someone gouged the center with a hollow circular object. Also, the quarter is no long round and looks a bit flattened from 9 to 12 o'clock on the obv. That process appears to have added some texture to the surface of the coin on both sides.

It's called Post Strike Damage ( PSD), not an error. The damage happened after it left the U.S. Mint (after the strike). Now it's only worth Silver melt value now.
