Looks like die crack on forehead and hair error die chip crack reverse.

Yep. A Die Crack and a Die Chip. The obverse die is displaying some Die Deterioration, also known as Die Fatigue.

Especially considering how low the relief has gotten. You would think the dies would last longer.

I'm not seeing Die Deterioration on the date area. So the die is just breaking down quicker, but has a lot more coins to strike yet.
