Makes me wonder that the opposite side of the coin looks like? If it is pushed out then it was PSD. If it is not pushed out it maybe a Struck Through Grease issue. I see a wave around the missing dot area and the void. The dot should be smaller than the void. So I'm thinking it was flatted and pushed out the opposite side.
