1961 LMC Defective Planchet?

I pulled this one a minute ago out of the same mint bag. RVS defect at "AM" of America. Any idea what may have caused this?

Looks like damage from a severe blow ( PMD).

But note the obverse? The rim is widened. I think the planchet was not complete before the strike. Note the outer edge is normal, just that area is affected.
