I think we're seeing Die Deterioration Doubling here. Note the flow lines toward the rim as well. Although not guaranteed, you're more likely to see hub doubling in the center of the design. Here are some links to give you an idea:

Also machine damage. You can have several issues on the same coin. Striking issues, storage issues, Die Deterioration issues, damage altering the devices. Also through in intentional damage as well. So it just have to be one thing that is an issue with a normal coin.
Same with variety coins as well.
