What might have caused this to be the only letter split like this on this 99P LMC?

Ejection Doubling. It can affect one or more letters usually in the direction that the coin was ejected making that mark.

Not eject doubling, there's no sharp edge. It's just a well paced coin hit.

What is a coin hit? Is it when another coin falls on that coin?

Not necessarily - it's when a coin is damaged by a hit from anything else.
