someone was playing around, it didn't leave the mint that way.. perhaps, they were going to put a jewel in there.

Damage yes, and although there is now way of knowing for sure, this damage almost certainly happened after it left the Mint.

It is not smooth on the sides inside it is rectangular and the P is flat could it have been from a chip from the die or from another coin?
