technical term would be a collar clash. the edge of the working hub struck the collar and picked up the reeding.
a lot of the collar clashes are visibly minor, like the 1922P Peace dollar VAM-2DY.
ANACS will even slab "error design creep", which is just Die Deterioration, so I don't think it being minor is the issue with it not being labeled as a collar clash.
Did you request it be slabbed as an error collar clash when you submitted it? It's really the only thing I can think of as to why they didn't put it on the label. they didn't know to look for it and attribute it.
And honestly it's pretty huge and is visible in the pictures from K1 to K3 then from K7 to K11 (clock positions)
Should be "1996 $1 Silver eagle"

only reason I can think of is that they flat out didn't know you wanted it attributed as a collar clash on the slab..
Not sure if this would constitute an "error" though, or like the Peace dollar mentioned, as a Die variety. Not sure if others have been found....
