This 1975 baffles me. The queen almost looks like she has a beak and if you flip it upside down you can see antlers and light outline of caribou. On the caribou side I can see crown of queen? So the coin was struck stuck to die fell off but flipped upside down and rotated 180° then struck over? Or a quarter was in machine instead of planchet and it was struck over quarter upside down and backwards?

Severe PMD, looks like it spent some time in some sort of acidic liquid at one point in its life, you can see that it is missing metal and this sort of damage wouldent happen at the mint IMO.
