I don't really see any errors, the coin has suffered a lot of damage though. The lettering and date have been flattened, and the copper plating has been worn from the rim.

@noob, I agree with @lcut that rather than being the result of a grease-filled die, the letters and other design features seem to have been flattened in circulation.

Judging by the look of the rim I'd say this spent a bit of time as a Dryer Coin.
