Damaged, well circulated nickel. Damaged (perhaps intentionally) after the coin left the mint.

Correct. Al altered coin is just a face value coin. When you go to the junkyard and see a wrecked car, that is not a factory error. No, the car when through an event that altered the looks of the vehicle. A damaged coin is not a mint error, as when the mint saw it last, it was normal.
