with john1. The wavy surface is caused by gas bubbles under the clad. The heat was high enough to partially melt the cupronickel layer and expanding gasses did the rest. The weight is a little off, but the major damage nullifies any possible value as it can't be attributed to a mint error and may have been wear on the coin before it was damaged.

If the coin was thrown in fire wouldn't the quarter be bent out of shape because it has no curves and it's flat or it would have more than one bubble right?

@guicho, this is a bit of speculation on my part, but the difference in thermal coefficient of expansion for the different metal layers seems to cause these bubbles. Sometimes there is only one and sometimes there are multiple.

So many variables going on. How long may it been heated? And how? blowtorch or campfire? Nobody knows. It is what it is.

Yeah, somewhere on this site, I have a 1966 dated 'campfire quarter' I found in as a coinstar reject.

With the different types of metal they don't adjust the same way with the heat:

The bubbles on your coin have been flattened, to be able to spend it in a vending machine.
