This looks like a punch thru or maybe damaged die on the rim. Or PMD.

Yes a little odd in that some of the metal from the rim got "smushed" onto the field, but I'm also in the damage camp on this one.

Displaced metal from the hit on the rim that's been worn and shaped by circulation. This smoothing of the metal has masked some of the features of the damage and made it look like something that was originally part of the design.
