On Reverse, and seeing the interaction with the bottom stem tip in "T" I believe this is a Struck Through.

This appears to be a stuck through something error. Note the ends are definite, and I don't see a wave happening on each side of the valley of where the event happened.

Regarding that visual aid... what IS that in the top photo? I've been trying to figure out "struck through" and the bottom image helps a lot.

I'm guessing that a 22 casing had been hammered into the reverse, leaving that damage.
