There is no way that could happen during the striking of the coin
so the only alternative is damage

An error would be something the mint did before/during the process. I use the term PSD. (Post-Strike-Damaged) To denote after the strike, damage would not be a mint process error. So all mutilation of coins are not errors. Just damaged. Just like going to a junkyard, and seeing a wrecked vehicle, would not be a factory error. It happened after the process of creation of vehicles. Even vehicles are damaged in shipping. Is that an error of a factory? No. The vehicle had left the factory. So PSD damage to a coin, starts after bagging the coins, or post strike.
