By swelled I mean the coin is thicker on both sides like it's double-plated or something in that nature there's a lot going on with this coin just not sure if this is common or not for this particular year

I'm totally going out on a limb and am going to guess that the nickel was in a fire or heated with a torch causing it to swell.

Please post a low angle photo to get a better look at the swelling. As for the steps, they've been largely obliterated due to circulation wear, including hits. Raised features on nickels of this era do take a beating.

Compare your coin's steps with these:

Not exactly razor sharp to begin with.

Note CENTS on the coin? The fields around the tops of these devices look like the fields were altered. Thus the design may have been altered on this coin on the die, and also looks like some contact mark on the coin with some incuse marks. Not sure why the dark color is present on this coin. But of the design alteration made the coin taller in profile, then the marks could be contact marks because of the extra thickness, that rims are not protecting. Interest in in this coin? I doubt if any would be created. It would be nice to see a view at an angle to see if the is extra height on that area of the coin?

1969-D-Nickel.-It's-Swelled-On-Both-Sides-Missing-Stairs-And-Other-Things here's a couple pics it seems like the swells are the same shape as the opposite side it looks like the capital on the obverse behind the head and it looks like the head behind the capitol on the reverse
