I'm not an expert by any means, but I will gladly give my humble opinion. Looks to me like a previously Encased Cent or some type of vise job? Damage for sure, not an error. Can't say exactly what kind of damage though.

Looks like it was altered. Note the alteration widened the coin making it appear as a partial collar. But a partial collar doesn't flatten both sides, only one. To me it looks altered PSD.
