Can anyone tell me what is going on with this coin? Is it a uniface coin? Or something happen to it? The reverse is very smooth with no ridges. I have no idea what the denomination would be.

That is a one cent "Vickie"
The lack of rim indicates that the reverse has been removed. PMD

Deliberately removed, likely by someone looking to make a love token...something popular in times past.
