1901-O 10c DDR? Tdr? MD?

I ran into this 1901-O Barber dime, PCGS MS63, tonight and am curious what you think this is. The mint mark is quite offset southwest on the left side, however the interior offset to the west doesn't match. The right ribbon shows tripling (second image), as does the leaf to the right. The left leaf (third image) also shows a bit of tripling. Overall you might note extremely weak strike in the upper right ribbon and upper wheat.

I can't decide whether this is a doubled/tripled die or machine doubling/tripling. It confuses me because I though the MM was punched into each working die during this era, so if it's a DDR/TDR then the MM was also punched wrong after hubbing the die.
