Large extra dies, is this an error?

This looks like a Die Chip. That section of the die broke and fell away leaving a hole. When the coin was struck the metal of the planchet was forced into that void.

I believe that lump is a die chip--not an 'extra die(s)'.

So it is not an extra die, it is a void on the die (where a piece broke off the die), which leaves a chip on the coin where the is void on the die.
