What would cause the penny to look like this, I was thinking off center and DDO possible, What would cause this to look like this?

Looks like a few things: 1. A Dryer Coin, 2. Die Deterioration from overuse, and 3. Circulation wear. None of the three carry any numismatic premium.

As undesirable as this coin looks, it is that much of an eyesore to a true coin collector. Just a Wheat cent now.
