I Am Wondering What May Have Caused This Damage? (1964 Kennedy Half Dollar)

Second, I agree that it is damage, but note that it is often quite hard to definitively know what specifically causes damage. It looks to me like the letter O took a hit which smeared some metal toward the letter D, but I have no idea of the circumstances. During the striking process, the metal discs that form coins are solid metal, but are "squished" in a die to make the designs. The damage that you are pointing out would have happened after that process. As you likely know, your coin is silver so definitely still worth saving.
