Looks like a lamination fell off before the strike, the reverse didn't have enough metal to fully strike up.

Possibly a defective planchet. Looks like cavity was there before it went through the upsetting mill.
This would also account for the weak strike on the reverse.
Would like to see the weight to two decimal points.
