All I can find is the engraver is likely I. Sato (Japanese) here is one of his medallions very similar style of engraving.

Trying to guess flags: United Kingdom, USA, Japan, France/Italy/Belgium? and I have no idea on the last one.

I'd guess from the overall design it's a peace medal or from a international treaty/summit event possibly. I doesn't look too old, need to find someone that reads Kanji/Japanese.

I can find that a "Mr. I. Sato" was the engraver to the Imperial Mint of Japan I think around late 1920's and on. I found it in an British Numismatic Society proceedings paper.

I'm currently recovering in hospital from a stroke and a scrambled brain but believe this to be a 1919 ish red cross medal.I have sold one.

The medal engraver has used a version of the Petra-Sancta hatching system for depicting colours on monochrome objects, though you have to rotate the medal 90 degrees to make the flags the "right way up" to make it work. The bottom two flags are Italy on the left (green, white, red stripes with royal coat of arms in centre) and France on the right (blue, white, red).

Thanks Sap! I figured it was something like that. I knew the flags were rotated 90 degrees but the Italy threw me as I've never seen that coat of arms like that displayed on the flag.
