Fairly nice example of this double error. The coin only weighs 2.43g so about 22% of it is missing from the clip. While a quick measurement puts the long (north-south) axis at 20.6mm so about 8% too wide. A thin pancake with a bite out of it.

Very cool. Wonder if the clip made the coin not seat right and part of it kept the collar from coming down?
